What happened
We pointed an assistant at eleven years of wiki pages. The first week it answered confidently from a deployment guide that had been wrong since 2021. There were four versions of the on-call guide and it picked a different one each time. The model was fine. The documents were the problem, and we had never had a reason to notice.
What it cost
Small: under €500 a month. The clean-up took two people most of a month.
What we fixed
Every page got an owner and a review date. Pages without an owner after 30 days were archived and left out of the index. We added 50 real questions with known answers and run them every week.
