Meeting notes bot: everyone liked it, nobody read it

What happened
We turned on automatic meeting notes for the whole department. People loved it in the survey. After two months we checked: most summaries were opened once, by the organiser. Action items were sometimes assigned to the wrong person, and nobody noticed because nobody read them. The meetings themselves did not get shorter.
What it cost
Included in a licence we already paid for, so the cost was attention, not money.
What we fixed
Notes now go only to the organiser, who checks and sends the action items by hand. It takes two minutes and the items are right.
